How we can promise a set price

In Arizona we run our own dentist network, called TDA, instead of renting one from someone else. We hold the contracts with the dental offices ourselves. That is why the copay on the schedule is the copay at the chair.

Preferred Care DHMO copays, effective January 1, 2026. Copay schedules are updated each January. Typical charge is an average of dental fees throughout the state. Copays for crowns and dentures include lab fees. Lab fees may vary, so ask your dental office.

Crowns, root canals and dentures are covered at these copays when your own general dentist does the work. Specialists are not covered on this plan, so the same work is not covered if it is sent to one.

Good to know

Specialists are not covered on this plan, in or out of network. That means endodontists, periodontists, oral surgeons and prosthodontists. At an in-network specialist you get 20% off and pay them directly. Many general dentists do crowns, root canals and dentures themselves, and that work is covered.

The full Arizona network is 4,171 dentists. Cleanings and exams twice a year. Crowns and dentures once every five years per tooth. Braces are not covered.

Going outside the network. With a general or pediatric dentist outside our network, EMI Health pays part of the bill and you pay the rest. Every amount is in the copay schedule.

Find your dentist

Each plan uses a different list of dentists. Open the one you are considering and search by ZIP code. The lists open in a new window, so this page stays put.

Common Questions

Is there a waiting period?

It depends on the plan. On the Preferred Care DHMO, no. Coverage works from your start date, including crowns and dentures. The Advantage plan waits three months for fillings and major work like crowns and dentures. The Premier PPO waits six months. Cleanings and exams are covered from day one on all three.

Does this work with other coverage I have?

Yes. These plans stand on their own and work alongside any other coverage.

What will a crown cost me?

$625 on the Preferred Care DHMO. Every procedure has a set copay you can look up before you book the appointment.

Can I keep my dentist?

Search the list for the plan you want. The Preferred Care DHMO lets you choose from 1,384 general dentists in Arizona. The Advantage plan has 2,773 general and pediatric dentists. The Premier PPO has 9,811 and also covers care outside the network.

What if I need a specialist?

Many general dentists do crowns, root canals and dentures themselves, and all three plans cover that work. If you need an actual specialist, it depends on the plan. The Preferred Care DHMO covers specialist care once the plan approves it first. The Premier PPO covers it. The Advantage plan does not cover specialists at all, though in-network specialists give you a 20% discount.

Do I file claims?

Not on the DHMO. You pay the set copay at the dental office and you're done.

Can I be turned down?

No. There are no health questions and no dental exam, and it doesn't matter what condition your teeth are in now. If you live in Arizona, your acceptance is guaranteed. Waiting periods on some plans still apply to certain services, and those are listed with each plan above.

Does my price go up as I get older?

No. Your age never changes your price. You pay the same rate at 45 or 75.
